Completing all of your patient forms before your initial visit will help speed the process of your initial registration with our office, and allows you to complete the forms at your pace in the privacy of your home. If you choose not to complete these prior to your initial visit, please arrive 30 minutes before your scheduled appointment time, as you will be required to complete these before seeing the physician. Please bring a copy of your insurance card, your driver’s license or photo ID, and be expected to pay your co-payment, co-insurance, or any deductible applicable to the visit.
